Diabetes Disease Management

Control Diabetes Services is dedicated to improving the lives of individuals with diabetes. We have established ourselves as the leader in face-to-face diabetes education and self-management training through our nationwide network of local education providers. We currently partner with 30 MCOs across the country.

We track over 400 data points and offer an on-line reporting tool to help our partners with NCQA / HEDIS requirements. We have been able to demonstrate clinically measured success and our results with diabetes patients speak for themselves.
