Each provider operates a diabetes program in conformance with standards of the American Diabetes Association. This network of qualified providers all utilize the Dia-Trac outcomes software system providing for uniformity of outcomes collection and reporting. The network assures market-wide geographic access for members of health plans served by Control Diabetes Services.

Outcomes Software System

All members of Dia-Net
® are licensees of Dia-Trac, the diabetes outcomes collection, analysis and reporting software developed by CDS. This unique software provides for uniform outcomes collection and reporting throughout the Network. All data are maintained in our National Diabetes Database, which allows for outcomes performance analysis of the Network as well as health plan reporting by site or on an aggregate basis.

Each member of the network provides diabetes training and education services in conformance with standards established by the American Diabetes Association. All education staff members of each network program must meet Control Diabetes Services' standards. In addition, each network member participates in our Continuous Quality Improvement program. Control Diabetes Services provides a toll-free member support line to assist network members in the operation of their program relative to network and contract management.
