| Each
provider operates a diabetes program in conformance with standards
of the American Diabetes Association. This network of qualified
providers all utilize the Dia-Trac
outcomes software system providing for uniformity of outcomes
collection and reporting. The network assures market-wide geographic
access for members of health plans served by Control Diabetes
Services. |

The
power of Dia-Net®
is the ability to provide comprehensive, high quality patient
education interventions for all members with diabetes in a Managed
Care Organization's geographic service area.
Whether your organization is a hospital, physician's practice,
home health agency, specialty provider or pharmacy, there is
an opportunity for you to expand into or broaden your current
capabilities in the field of diabetes patient education.
Control Diabetes Services has a successful history of contracting
with the managed care industry, including a current portfolio
of over 30 managed care contracts covering approximately 47
million lives.
As a member of Dia-Net,
you will participate in the benefits of serving this population
as well as future customers as we aggressively market the Network
to local, regional and national MCOs.
Participation in Dia-Net opens opportunities for managed care
reimbursement to providers who would otherwise not have the
capacity to serve MCO membership over a broad geographic area.
MCOs need population-wide geographic coverage of their members.
Access is important, quality is important. Dia-Net is the answer
and your organization can be a part of this dynamic Network.
